I have worked in most of the places an adult with a communication or swallowing problem ends up. Hospitals, where you learn to move quickly, read a chart properly, and recognise when something needs escalating today rather than next visit. Skilled nursing and assisted living, where you learn how much of a person’s day is shaped by staff who have four minutes. And homes, where you learn what someone actually needs on a Tuesday afternoon.
Those settings teach different lessons, and the home is where they come together. A goal that looks reasonable on a treatment plan can be useless in a kitchen. A strategy that works in a quiet therapy room can fall apart at a family dinner. Working in the home means the plan gets tested against real life every single week.

How the practice works
Healthcare Straight Up is a solo practice, and that is deliberate. The person who evaluates you is the person who treats you every week. Nothing is handed to a rotating therapist, nothing is lost between visits, and I carry the whole picture of what we are working on.
Most referrals come from physicians I have worked alongside for years in this area. Families reach out directly too, and neither route is better — the first conversation looks the same.

Beyond the caseload
I write and sell clinical materials for other therapists — goal banks, printable activities, and a dementia reference guide used by speech, occupational, and physical therapists working in skilled nursing. Good clinical tools should not stay locked inside one clinic.
